Player Loan Form
Use this form to loan a US Youth Soccer-registered player for a specific competition from one US Youth Soccer club/org to another. See the instructions on the form for more information.
Player Information, Medical Treatment Authorization, Liability Waiver/Release and Consent Form
The US Youth Soccer Medical Treatment Authorization is a legal form you must complete for your child to participate in US Youth Soccer programs, authorizing medical care and release from liability.
Purpose
- Authorizes athletic trainers, coaches, EMTs, physicians, nurses, dentists, and other healthcare providers to treat your child during soccer activities.
- Releases US Youth Soccer, its member organizations, sponsors, and related personnel from liability for injuries or illnesses occurring during participation.
- Authorizes emergency transportation to a medical facility if needed.
- Requires proof of a recent physical examination by a licensed doctor confirming your child is medically fit to play.
